Tina Karwajaltys presented the results of the C‐CHAP Project (Cardiovascular Health Awareness Program). This was a randomized controlled trial rolled out to 39 medium‐sized communities across Ontario in the Fall of 2006. Patients over the age of 65 were invited to attend two CHAP sessions where volunteer peer health educators assisted with blood pressure measurement, CVD/stroke risk profile and in handing out educational materials and linking patients with community resources.
The goal of this program was to reduce the annual mean rate of hospital admissions for stroke, acute MI, and CHF (composite score) for residents 65+, using routinely‐collected, population‐based administrative health data.
