  • Congratulations to the Integrating family Medicine and Pharmacy to Advance primary Care Therapeutics (IMPACT) Project Team (Lisa Dolovich, Principal Investigator) for receiving a certificate of excellence from Blood Pressure Canada.

  • Congratulations to the Vascular Intervention Program (Lisa Dolovich Co‐Principal Investigator), a project led by the Group Health Centre in Sault Ste Marie for receiving an Ontario Hospital Association Leading Practices Award at the OHA HealthAchieve 2007 conference.
  • Cardiovascular Health Awareness Program: The CHAP team held an afternoon session at the Stroke Strategy meeting for Southwestern Ontario ‐ held in London on November 7; this was a timely opportunity to bring together key partners from our 20 program communities across Ontario to discuss plans for the next phase of CHAP, funded by the Ministry of Health Promotion ($500,000). The ‘CHAP + Action Plan’ project will focus on sustainability of CHAP in the communities, enhanced components to provide the ‘next steps’ after the pharmacy‐based sessions, and working with
    FHTs to integrate CHAP with their chronic disease prevention activities and create a model for wider implementation.
