On-line Abstract Submission Form

Abstract Submission Date Extension: May 19, 2006

Thank you for using the Trillium On-line Abstract Submission Form. While we encourage abstract submissions that present completed research, one of the goals of the Trillium conference is to offer a supportive environment for investigators to air their ideas. As such, we welcome abstracts that describe preliminary data or work-in-progress. We also invite submissions from a broad range of primary health care professionals, nurses, residents, and students.

Free-standing paper presentations will be 15 minutes in length. The first 10 minutes will be alotted for presentation of your work. The remaining 5 minutes will be reserved for questions and discussion.

Please fill out the information below and finish by clicking the "Submit" button. Your abstract will be sent to the conference planners. You will be contacted confirming receipt of your submission. As the meeting date approaches you will be sent details regarding the time and location of your presentation. Please remember to also register as participant here.

You are also welcome to FAX your abstract to Trillium 2006 at 905-527-4440 or mail to:
